To set up new Account you can either:

  1. Complete the below Fields and Upload completed “Card Authorization Form”

  2. Download and print “Card Authorization Form” and “New Account Form” and bring completed forms in person to our office at 1213 S. Clodfelter Rd, Kennewick WA 99338
